By capturing a Requisition you are logging the request submitted by the Doctor or other health professional. It is extremely important to record all information here accurately. Incorrectly recording patient demographic data in particular can place a patients life in danger as reference ranges may be incorrect.

Fields

• Callout Ticket Details: Enter the callout ticket number if applicable. This will allow you to import information from the callout that was logged. Such information may include patient details, doctor details and even scanned copies of the form which the courier may have collected.
• Card Number: If your organisation implements a loyalty programme, enter/scan the patients card number here.
• Collected By/On: Enter the logistics user that collected the form as well as the date/time it was collected
• Received By/On: Automatic field generated from login credentials

• Requesting Doctor Mnemonic: Enter the Doctors Mnemonic. Recall that the Mnemonic allows the LIMS to easily identify the origin (both Doctor and Practice) using this key
• Copy Doctor: The mnemonic of the Doctor that requires a copy of the result report
• Requisition Form Number: The paper form number
• Originating Requisition Form: If this is a multi-part Requisition enter the first parts Requisition number.

• Search: Enter the Guarantors first name, last name or ID number to search for an existing guarantor
• Loyalty Number: Enter the loyalty number of the Guarantor if applicable
• ID Number: The social security ID number
• Passport Number: The international passport number
• Title: The title by which the Guarantor is addressed
• First Name: The first name of the Guarantor
• Last Name: The last name of the Guarantor
• Physical Address: The physical address of the Guarantor
• Postal Address: The postal address of the Guarantor
• Telephone No (h): The home telephone number of the Guarantor
• Telephone No (w): The work telephone number of the Guarantor
• Telephone No (c): The cell phone number of the Guarantor
• E-Mail: The e-mail address of the Guarantor
• Payment Mode: This drives the Tariff sheet selection.
  • Cash (Dr): Choose this option if the Doctor is being billed. Pricing will be determined based on the Doctors profile. The Tariff sheet must be setup accordingly for the Doctor
  • Private (Subsidiary): Choose this option if the Guarantor is being billed directly. Pricing is determined in the Subsidiary configuration for the Branch where the Requisition is logged.
  • Medical Aid: Choose this option is the Guarantor is paying via Medical Aid
  • Company: Choose this option if a Company or Corporate account is being billed.

• Search: Enter the Patients first name, last name or ID number to search for an existing Patient
• Loyalty Number: Enter the loyalty number of the Patient if applicable
• ID Number: The social security ID number
• Passport Number: The international passport number
• Title: The title by which the Patient is addressed
• First Name: The first name of the Patient
• Last Name: The last name of the Patient
• Date of Birth: The date of birth of the Patient. Click the refresh button next to the Date of Birth input field to update the Date of Birth based on the ID number (South Africa)
• Sex: The sex of the Patient
• Race: The race of the Patient
• Telephone No (h): The home telephone number of the Guarantor
• Telephone No (w): The work telephone number of the Guarantor
• Telephone No (c): The cell phone number of the Guarantor
• E-Mail: The e-mail address of the Guarantor

Hospital

• Hospital: The Hospital that the Patient is admitted to. Not Hospital requisitions are automatically marked urgent.
• Ward: The ward that the Patient is admitted to
• Patient File Number: The Patient file number

Chronic Conditions

• ICD10 Code: Select one or more ICD10 code. Type to search, then click the Add button. To remove an ICD10 code, click on the blue link that appears once its been added.

Clinical History

• Enter the Patients clinical history here. It is printed on the result report.

• Test Profile: Type a Test Profile to search. Click it to select. To remove it, click the blue link which appears after its been added.
• Test: Type a Test to search. Click it to select. To remove it, click the blue link which appears after its been added.

NOTE: You will only be able to select Tests and Profiles which have been activated on the Tariff sheet selected. Refer to the Guarantor tab Payment Mode (further up)

Specimen

• Specimen Taken By: The name of the person that took the specimen. Does not need to be user on the system.
• Specimen Taken On: The date and time the specimen was taken

Tubes

• Tube Type Barcode: Select the Tube Type or scan the tube (Consult with THASA for more information on how your LIMS can be configured for this)
• Organ: For Histology, enter the organ that the specimen was taken for.
• Side: Choose None for clinical results. Left or Right for Histology where applicable.
• Specimen Barcode: Enter the specimen barcode or scan the tube. Click the Add button to add the specimen.

• Internal Comments: Internal comments visible only to laboratory staff
• External Comments: External comments visible to the Doctor and Patient on the result report

• Add Attachment Button: It is recommended that you save a scanned copy of the hard copy form you received. You can do this at any time.
• Various check boxes: Before saving the Requisition all information must be validated.
• Reason: When editing a Requisition, the reason why the Requisition was edited.
